Requesting Your Medical Records
You have the right to view and/or request copies of your medical record. To view your medical record, you must make an appointment with the Health Information Management (HIM) Department. Copies are provided only after the patient is discharged. Please allow 30 days from the date of discharge for record processing before copies will be available.
You may obtain all your labs and radiology reports on the patient portal. The patient portal is a secure online website that gives you convenient 24-hour
access to your personal health information and medical records—called an Electronic Health Record or EHR—from anywhere with an Internet connection.
The Patient Portal allows access to your medical record to view your 1) the providers that have given you care; 2) health problems; 3) allergies; 4) immunizations; 5) medications; 6) in-house lab test results; 7) vital signs and 8) radiology reports. These can be downloaded and shared with other providers.
During admissions, a SRMC staff member will record your email address to create your access to the patient portal. An automatic email is sent to you from firstname.lastname@example.org to invite you to access your patient portal. For assistance with accessing your patient portal contact the patient portal support team or click the following link to access your patient portal.
To receive copies of your medical record, submit a completed authorization form. This form must be signed and may be submitted by fax, mail, email, or in person. Verification of identity is required. Acceptable forms of identification include driver’s license, employment ID, state issue ID, current school ID, military ID, V.A. ID or a valid, current passport.
An authorization form may be picked up and dropped off at Sidney Regional Medical Center at any admissions area between the hours of 7am to 5pm Monday - Friday or at the Osage Campus, 1625 Dorwart Dr, Sidney NE, Monday - Thursday 8am - 5pm and Friday 8am - 4pm.

If you are requesting copies for someone other than yourself, you will need to provide legal documentation verifying legal guardianship, power of attorney, executorships, or next-of-kin relationship of a decedent. Parents may request copies of their minor child’s records if they have legal custody of the child and the child is not legally emancipated.
Note: The HIM and Medical Records Departments do not have access to radiology or other imaging films or billing information. Please contact the respective departments for those records.
Release of Information cannot review, discuss, or interpret medical records. Please direct these questions to your healthcare provider.

Acrobat Reader will allow you to complete most of the form's required information on your computer. However, you must print out the form and bring it to SRMC to be signed, dated, and witnessed. You will not be allowed to save the form to your computer, so please be sure your information is correct before printing it out.
There is no charge for requesting a copy of your medical records for personal use or for medial provider use.
