What is Complex Illness Care? Care that supports improving quality of life for patients with complex and chronic illnesses that may be provided at the same time as other medical care in collaboration with your primary care provider.

• Adult patients only

• Self-refer and/or provider referrals accepted

• Initial assessment for the complexity of illness and creation of care plan

• Collaboration with primary providers

• The provider will recommend the next steps during illness regression
